Death Certificate

A death certificate is filed in the Town Clerk's office if the deceased was pronounced dead within the limits of the Town of Black Brook.  The funeral director is responsible for filing the Death Certificate with the Town Clerk.  Certified copies can be requested for $10 each.

How to Obtain A Copy of a Birth or Death Certificate?

Certified copies of Birth and/or Death certificates can be requested in person at the Town Clerk’s Office at 18N Main Street, Au Sable Forks, NY 12912. Monday, Wednesday, Thursday, 9:00 AM to 2:00 PM and Tuesday, 12 PM to 5PM.

Identification Requirements: 

One (1) of the following forms of valid photo-ID:

• Driver license
• State issued non-driver photo-ID card
•  Passport
• U.S. Military issued photo-ID

Two (2) of the following showing the applicant’s name and address:

• Utility or telephone bills
• Letter from a government agency dated within the last six (6) months
